BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: November 1, 2016 TO: Honorable Chairman and Members of the Board FROM: Zan Fedorak SUBJECT: Request approval to revise Task Order #3 with Matrix Design DEPARTMENT: Purchasing BCC DISTRICT: ALL STATEMENT OF ISSUE: Request approval for the Purchasing Manager to correct Task Order (TO) number by striking through TO #3 and revising to reflect TO #4. BACKGROUND & ANALYSIS: On October 18, the Board approved TO #3 for Capital Improvement Opportunities for ECCC, Matirx Design Group, contract C14-2157-TDD. After the Board approved, it was found that TO #3 should have been noted as TO #4. Purchasing is requesting that the Board give the Purchasing Manager authority to strike through TO #3 and revise to reflect TO #4 on the document. OPTIONS: Approve/Deny approval for the Purchasing Manager to correct TO number by striking through TO #3 and revising to reflect TO #4. RECOMMENDATION: Staff recommends approval for the Purchasing Manager to correct TO number by striking through TO #3 and revising to reflect TO #4.
