2002 54- Justification Su in7tary A. Problem Description: In 2042 the City of Fort Walton Beach Fire Department FWBFD) started providing advanced life support to our .first response and neighboring districts. The FWBFD had previously only provided basic life support. The department now provides Paramedics on all medical calls. The reason for this matching grant is to purchase Simulation manikins for training our Paramedics to the highest level in airway standards. These manikins are Advanced Life Support Manikins iNdth airway management heads and CPR manikins are for basic life support skills. Curently the department has no manikins or simulated airway devices and has to rely on a coordinated effort to use the local county EMS and college equipment, which is outside of our normal response area making it difficulty to train on duty. When training in airway management, CPR, or other emergencies members currently are unable to simulate difficult or complicated airway procedures and must rely solely on diagrams in training manuals or availability of the previously mentioned options. Since training is an important component of efficiency, competency and patient outcome, each member of the department needs to have these important resources available. B. ' resent Situation: Currently the situation is such that each member of FWBFD is given (2) hours of training a year by PowerPoint in airway management. The only time actual hands on with manikins is during the required ACLS, PALS, and CPR recertification classes in airway management. In addition to the two hours, the member has a 2. 5 hour online training class in airway management each calendar year. The only time the member is actually hands on with a manikin is during the required ACLS, PALS, and CPR recertification classes offered by neighboring departments during the 55- two ( 2) year recertification cycle. As a result 30% to 40% of intubations attempts are not successful on the first attempt due to technique problems and lack of hands on training. Paramedics are grossly under trained in the management of difficult airway, complicated airway, or for that matter any airway. C. Proposed Solution: Upon award of this grant, the FWBFD will immediately purchase the advanced life support adult and pediatric Simulaids© for airway management and the basic life support CPR adult and pediatric manikins for CPR training and recertification. Our members will train two (2) hours per month with these devices in simulated airway management, mock code scenarios, CPR. training and recertification and myriad of other evidence based scenarios. Our Paramedics will become better trained, more efficient, and confident in managing the emergency care scene." Consistent training and communication among healthcare providers improves outcomes for seriously injured or ill patients." This statement is the hallmark of the American Heart Association. D. Consequence of not funding: Without this grant the FWBFD cannot purchase this much needed training equipment and the percentage of successful intubation' s attempts will remain lower than they should. Statistics have shown that with frequent training the percentage of failed attempts will decrease and the survivability of patients will increase. On average the FWBFD runs 6 to S cardiac arrest or respiratory arrest calls a month which require intubation. The level of training our members are receiving now is grossly inadequate due to the lack of proper equipment and because of this point alone I believe the public is at risk. When a Paramedic is trained and tested the overall public benefits from the skills obtained and the confidence gained. 56- E. Geographic area to be addressed: The Fort Walton Beach Fire Department is a 37 member department on the Northwest Florida Gulf Coast. The district i.s a 25 square mile area with an approximate population of 25, 000 during the normal season and up to 50, 000 during the peak tourist season, which is a 100% increase in population. In the last 5 years the FWBFD has increased the Paramedic manning from 12 to 15 allowing the addition of a non transport ALS rescue truck. This is due to the increasing number of medical calls. On average the call load has increased 10 to 20 °% o for medical calls each year. With the growing population of elderly citizens in the is area it will only continue to increase. In 2008 the FWBFD received 2946' emergency 911 calls of which on average 80% were medical related emergencies. F. Proposed time frames: The proposed time frame for the purchase, implementation, and training of the equipment with the approval of this grant is to: 1.) Within 30 days of award, place the order for the equipment. J. 2.) Take receipt of equipment within appropriate shipping period of six (6) weeks after purchase. 3.) Develop a training program in conjunction with the Medical Director and put the equipment in service within two (2) weeks of equipment arrival. G. Data Sources: The data obtained herein has been quoted or paraphrased from several sources including The Okaloosa County Emergency Dispatch Center. The American Heart Association: ACLS for Experienced Providers and American Heart Association: Pediatric Advanced Life Support. Also some of the data was obtained from our geographical data information, run reports, and local county government reports including the jurisdictional run data reports. 57- H. Attestation Statement: This proposal is a new grant project and does not duplicate any previous project or proposal under this, or any other grant program. 58- Outcome for Training Projects A. How many people received training in most recent 12 months period: Since the inception of the Advanced Life Support program which placed Paramedics on our first response Engine and rescue Companies in 2002, 15 Paramedics have had minimal hands on training in intubation which they received during recertification periods with coordinated efforts of our local Eras agency and Colleges. None of our individuals have been able to train on a monthly bases using advanced life support SimulaidsO or basic life support manikins. B. How many people do you estimate will successfully complete training in the 12 months after training begins: 36 people will absolutely receive the training during the first 12 months of receiving this grant under this proposal, which includes the 1 Fire Chief, 9 Fire officers and 24 Firefighters of which 15 are Paramedics and the rest EMT' s, as well as the 3 fire departments that provide mutual aid to the FWBFD response area. Those departments include Ocean City "Wright Fire District which currently has 6 Paramedics and several EMT' s, Mary Esther Fire Department which has 12 EMT' s and Okaloosa Island Fire Rescue which has 7 Paramedics and several EMT' s. C. Impact Data for 12 months before and 12 months after project: The impact this project will have most assuredly increase the success rate of intubation to hopefully 100% success on the first attempt and save more lives. The American Heart Association clearly states that, " Training and communication among healthcare providers improves the outcome of seriously ill or injured patients." Therefore with the approval and implementation of this project, lives will be saved through active training and improved communication. 59- D. Explain the derivation of all figures: The date obtained herein has been quoted or paraphrased from several sources including the American Heart Association: A.CLS for Experienced Providers and American Heart Association : Pediatric Advanced life Support. Local Department Data is a close estimate due to changing in manning and Retirement' s. E. integrated 55 year plan: This project, when approved, will be integrated into our five 5) year plan by allowing all personnel access to important training adjuncts. The training grogram will be designed with two ( 2) hours per month dedicated to training in i airway management and basic life support. Approval of this project will give the department an important training tool that can be built into a lifelong learning approach related to the adult airway management, pediatric airway management, advanced life support care of the adult and pediatric and basic life support and CPR. recertification and training. The members of this department will become more proficient and confident during the five ( 5) year plan in the maintenance and management of difficult pediatric and adult airways. The training aids will be utilized during the five ( 5) years following this grant project approval and beyond. 60- Statutory Consideration and Criteria A. Serve the requirements of the population on which it will impact: Section 401. 113( 2)( b) requires that grant money be used by organizations for the purpose of " increasing existing levels of emergency medical services." The proposal contained herein requests the funding for adult and pediatric advanced life support and basic life support training SimulaidsO and manikins. The training aids when purchased and utilized will improve patient outcome therefore increasing the level of emergency care as documented in the Florida Statute. The increase in training will benefit the public through active training and communication. B. N/A C. N/A D. N/A E.
