This program is designed to drive participation and engagement in wellness activities and increase utilization of the Florida Blue-sponsored tools and channels. Reward Type: Employer Specific Reward Description: 50 points equal $50. A maximum of 150 points can be earned which equals $150. Redemption: Within program effective end dates Program Eligibility – all active full time employees Reporting Requirements – Frequency: Quarterly Type: Standard Get Assessed Reported Activity Points Recurrence Activity Period Timeline Better You - onsite biometric screening– complete All employees can participate in the onsite biometric screening. 20 Once Jan – Feb 4-6 weeks Note: Registration is required. Quest BluePrint for Health Patient Service Center - biometric screening - complete Employees that are contract holders/subscribers on the Florida Blue health plan unable to attend the onsite biometric screening may do their screening at a Quest Patient Service 20 Once Jan - Feb 4-6 weeks Center (PSC) between January 19 - February 28, 2017. Note: Registration is required. Risk Management will provide the log-in process and Registration Key. WebMD Personal Health Assessment/ online The online WebMD PHA is available for completion by all employees. You can choose to complete the WebMD PHA more than once; however only the first PHA completed between 20 Once Jan - Feb 4-6 weeks January 19 – March 10, 2017 will earn you points. BMI- ideal (18.5 - 24.9) 20 Once Jan - Feb 4-6 weeks BMI can only be assessed at a scheduled screening between January 19 - February 28, 2017. Please allow 4-6 weeks for points to load. Cholesterol ratio- ideal (5.0 or less) 20 Once Jan - Feb 4-6 weeks Cholesterol can only be assessed at a scheduled screening between January 19 - February 28, 2017. Blood pressure- ideal (less than 120/80) 20 Once Jan - Feb 4-6 weeks Blood Pressure can only be assessed at a scheduled screening between January 19 - February 28, 2017. Please allow 4-6 weeks for points to load. Non-tobacco user 20 Once Jan - Feb 4-6 weeks Non-Tobacco user status can only be assessed with the WebMD PHA completed between January 19 – March 10, 2017.
