Walton Beach, FL 32547 hereinafter referred to as Agency. WITNESSETH: WHEREAS, the Department and the Agency heretofore on the 6th day of December 2011 entered into a Joint Participation Agreement; and WHEREAS, the Agency desires to accomplish certain project items as outlined in the Attachment "A" appended hereto; and WHEREAS, the Department desires to participate in all eligible items for this project as outlined in Attachment "A" for a total Department Share of $30,000.00 NOW, THEREFORE THIS INDENTURE WITNESSETH: that for and in consideration of the mutual benefits to flow from each to the other, the parties hereto agree that the above described Joint Participation Agreement is to be amended and supplemented as follows: 1.00 Project Description: The project description is amended To provide commuter assistance funding for the operation of alternative commuter transportation for the route from North Okaloosa county the the Ft. Walton Area. 725-030-07 PUBLIC TRANSPORTATION Page at 4 2.00 Project Cost: Paragraph 3.00 of said Agreement is increased by $20,000.00 bringing the revised total cost of the project to $60,000.00 Paragraph 4.00 of said Agreement is increased by $10,000.00 bringing the Department's revised total cost of the project to $30,000.00 3.00 Amended Exhibits: Exhibit(s) 8 of said Agreement is amended by Attachment "A". 4.00 Contract Time: Paragraph 18.00 of said Agreement September 30th 2018 5.00 E-Verify Vendors/Contractors: 4. Fund Participation: As Approved As Amended Net Change Department: $20,000.00 $30,000.00 $10,000.00 Agency: $20,000.00 $30,000.00 $10,000.00 $0.00 Total Project Cost $40,000.00 $60,000.00 $20,000.00 Comments: RESOLUTION NO. 13- WHEREAS, the Florida Department of Transportation, hereinafter referred to as the DEPARTMENT has prepared a Supplemental Joint Participation Agreement No.2 to provide funding for the operation of alternative commuter transportation to the Okaloosa County Board of County Commissioners, hereinafter referred to as the Agency, and WHEREAS, the DEPARTMENT has been authorized under Chapter 341.041, Florida Statues, to enter into the Supplemental Joint Participation Agreement No.2 and agrees to a Department participation of $10,000, and WHEREAS, the AGENCY has the authority to enter into the Supplemental Joint Participation Agreement No.2 and to undertake the project. The AGENCY further agrees to the conditions upon which assistance will be provided and the understanding as to the manner in which the project will be undertaken, as outlined in the Joint Participation Agreement. Now Therefore, the AGENCY authorizes the Chairman to execute the Supplemental Joint Participation Agreement No.2 DULY PASSED AND ADOPTED in regular session this 1* day of October, 2013.
