effective July 1st 2025. Public Content The Benefits Oversight Group (BOG) has recommended that the District contract with LEAP Health to provide a tool that our medical plan participants can utilize to help coordinate care for infusions and injectables delivered in an in-home setting by licensed practitioners. Members engage with LEAP Health directly to coordinate this care to maintain HIPAA protection. It will be patient choice if they choose to go with LEAP Health's recommendations and care coordination. Services include but are not limited to the following: ● Data intake and analysis: Leap Health to receive regular claims files from TPA or Client to identify eligible claimants after the initial prior authorization has been approved. ● Outbound engagement: Leap Health Care Guides to engage eligible plan members to coordinate and schedule homecare for approved and covered infusion therapies. ● Drug procurement: Leap Health will manage a network of specialty pharmacies for the procurement of specialty infusion therapies and required DME at the lowest price available to the plan. Leap will order the drug and coordinate shipping with the patient to their home. ● Care coordination: Leap Health Care Guides will work with plan members, accredited home infusion nurses and agencies for coordination of care to be performed in the home at a time that is convenient for the member. ● Site of service redirection: in the event that a patient does not want to receive care in the home, Leap Health will refer the patient to an in-network infusion center as an alternative low-cost option for receiving care. ● Non-infusion therapies: in such circumstances that TPA shall engage Leap Health in the procurement of non-infusion specialty medications, Leap Health will work with their specialty pharmacy network to coordinate drug procurement through the best partner possible. ● Reporting: Leap Health to provide ongoing utilization reporting and analysis to Client. Additionally, Leap Health will provide utilization reporting in an ongoing fashion to the prescribing provider. For additional information, please contact Sarah Cato at 850-833-5823.
