(The Environmental Quality Company) is pleased to submit this quotation letter to Okaloosa County Recycling. This quotation and all work performed subsequently are governed by the Hazardous Waste Disposal Services contract (contract # C14-2099-PW). If accepted, the following pricing can serve as an addendum to the contract. Contract Price Structure Disposal Category Quantity Rate Bulk Flammable Liquids Toxic 85 gallon drum $330.00 Bulk Flammable Liquids Toxic 55 gallon drum $300.00 Bulk Flammable Liquids Toxic 30 gallon drum $225.00 In the event that the scope of work should change there will be additional charges; these will be quoted separately. This quotation includes transportation and disposal and is contingent upon the waste conforming to the approved profile. Al containers must meet D.O.T. requirements. This serves as notice as required by 40 CFR 264.12(b) that EQ Florida, Inc. has the appropriate permits and will accept the approved waste streams.
