BOARD OF COUNTY COMMISSIONERS
AGENDA REQUEST
DATE: November 20, 2012
TO: Honorable Chairman and Members of the Board
FROM: John Hofstad
SUBJECT: Pocahontas Drive - Crawford Drainage Easement
DEPARTMENT: Public Works
BCC DISTRICT: 4
STATEMENT OF ISSUE: Okaloosa County Road Department recently repaired the failed
drainage pipe located in the north ten feet (10’) of property owned by Ronald and Susan
Crawford. This drainage system collects stormwater from the county road known as Pocahontas
Drive and conveys it to Mills Bayou.
BACKGROUND AND ANALYSIS: The drainage system lying along the north 10 feet of
property owned by the Crawford’s has been in existence for many years. Based on a search of
the records it has been discovered that this system was installed by the developer of the Colony
Estates Subdivision and an adjacent landowner. A drainage easement was created and recorded
between the respective parties for the infrastructure and the maintenance of which was to be the
developer's responsibility, Valleybrook Developers, Inc.
This drainage system collects and conveys county stormwater from Pocahontas and Alden Road;
however, the county never has officially been granted a drainage easement cover the existing
infrastructure.
During the most recent repair to this drainage system, the staff requested that Mr. Ronald
Crawford grant the Board of County Commissioners a drainage easement to facilitate the future
maintenance of this system.
OPTIONS: Approve/Disapprove acceptance of the Easement
RECOMMENDATION: Approve the Easement for recording and acceptance into the County’s
permanent maintenance system.

AGENDA REQUEST
DATE: November 20, 2012
TO: Honorable Chairman and Members of the Board
FROM: The Pancreatic Cancer Action Network
SUBJECT: Pancreatic Cancer Awareness Proclamation
Request approval of proclamation designating the month of November as Pancreatic Cancer
Awareness Month.
Packet Pg. 72
7.4.a
PROCLAMATION
Attachment: Pancreatic Cancer Awareness Proclamation 2012 (1147 : Pancreatic Cancer Awareness Proclamation)
WHEREAS, in 2012 an estimated 43,920 people will be diagnosed with pancreatic
cancer in the United States and 37,390 will die from the disease and pancreatic cancer is one of the
deadliest cancers, is the fourth leading cause of cancer death in the United States, and is the only
major cancer with a five-year relative survival rate in the single digits at just six percent; and
WHEREAS, when symptoms of pancreatic cancer present themselves, it is usually too
late for an optimistic prognosis, and 74 percent of pancreatic cancer patients die within the first
year of their diagnosis while 94 percent of pancreatic cancer patients die within the first five years
and of all the racial/ethnic groups in the United States, African Americans have the highest
incidence rate of pancreatic cancer, between 34 and 70 percent higher than the other groups and
approximately 2,670 deaths will occur in Florida in 2012; and
WHEREAS, there is no cure for pancreatic cancer and there have been no significant
improvements in survival rates in the last 40 years and the federal government invests significantly
less money in pancreatic cancer research than it does in any of the other leading cancer killers; and
pancreatic cancer research constitutes only approximately 2 percent of the National Cancer
Institute’s federal research funding, a figure far too low given the severity of the disease, its
mortality rate, and how little is known about how to arrest it; and
WHEREAS, the Pancreatic Cancer Action Network is the first and only national patient
advocacy organization that serves the pancreatic cancer community in Okaloosa County and
nationwide by focusing its efforts on public policy, research funding, patient services, and public
awareness and education related to developing effective treatments and a cure for pancreatic cancer
and the Pancreatic Cancer Action Network and its affiliates in Okaloosa County support those pa-
tients currently battling pancreatic cancer as well as those who have lost their lives to the disease
and are committed to nothing less than a cure.
NOW, THEREFORE, BE IT RESOLVED by the Board of Commissioners of Okaloosa
County, Florida, that we do hereby proclaim November 2012 as
PANCREATIC CANCER AWARENESS MONTH
DULY PASSED AND ADOPTED this 20th day of November, 2012.

BOARD OF COUNTY COMMISSIONERS
AGENDA REQUEST
DATE: November 20, 2012
TO: Honorable Chairman and Members of the Board
FROM: James D. Curry
SUBJECT: Tri-County Community Council
DEPARTMENT: County Administrator
BCC DISTRICT: All
STATEMENT OF ISSUE: Request approval of Abra McGill of Growth Management as the
Okaloosa County representative on the Tri-County Community Council Board of Directors for
the 2013 term.
BACKGROUND: The Tri-county Community Council is a community action agency serving
Bay, Holmes, Jackson, Okaloosa, Santa Rosa, Walton and Washington counties. The council is
required to have a Board of Directors with representatives from government, business and low-
income areas. Although a county commissioner often fills the role of representative to the
council, the by-laws allow the Board to appoint a designee to represent the commission. Ms
McGill fulfilled that role in 2012 and has agreed to do so again in 2013. The Department of
Community Affairs requires that the Tri-County Community Council receive all appointments in
writing. Letter of appointment is attached.
OPTIONS: Approve or disapprove.
RECOMMENDATIONS: Appoint Abra Mcgill as the Okaloosa County representative on the
Tri-County Community Council Board of Directors.
