There are several ways to access the guidelines: * From the Division of Library and Information Services’ website * By mail request to Grants Office, Division of Library and Information Services, R.A. Gray Building, 500 S. Bronough St., Tallahassee, Florida 32399-0250 * By email request to grantsoffice@dos.myflorida.com * By phone request to 850.245.6620 * By fax request to 850.245.6643 You must complete the application online through the Florida Libraries and Grants system. The deadline for application submission is 11:59 p.m., October 1, 2015. The grant agreement has been revised this year to reflect current Department of State and Department of Financial Services requirements. Please note that one item that is referenced on the grant agreement – “ATTACHMENT B – Fiscal Year 2015-2016 State Aid to Libraries Final Grants” – will not be attached to the agreement until after the grant amounts are calculated in December 2015 and the grant is signed at the Division.
