OKALOOSA COMMUNITY DEVELOPMENT CORPORATION
for
UTILIZATION OF THE STATE OF FLORIDA STATE HOUSING INITIATIVES
PARTNERSHIP PROGRAM or (SHIP)
This Amendment No. 3 is made and entered into this _ 16th day of June 2015 by and
between OKALOOSA COUNTY, Florida (hereinafter referred to as "County or Recipient") and
OKALOOSA COMMUNITY DEVELOPMENT CORPORATION, (hereinafter referred to as
"Subrecipient"). '
WHEREAS, the County has received grant funding from the State of Florida State Housing
Initiatives Partnership Program or (SHIP); and
WHEREAS, the County entered into a Subagreement #C11-1900-GM on August 16, 2011 with
Subrecipient to perform certain tasks as so specified in the Subagreement and amended with
Amendment No. 1 on November 5, 2013 and Amendment No. 2 on March 4, 2014.
NOW, THEREFORE, in consideration of the mutual promises set forth below, the County and
the Subrecipient agree to amend the Subagreement as follows:
ARTICLE II. Period of Performance
Is hearby amended to reflect an end date of June 30, 2016.
Attachment A.
Attachment “A” to Subagreement #C11-1900-GM is hereby removed and replaced with
the document attached hereto as Exhibit “1” to this Amendment, and shall be known as
“Attachment A” to Subagreement #C11-1900-GM.
All provisions not in conflict with this Amendment remain in full force and effect, and are to be
performed at the level specified in the Subagreement.
