Dear Board of Commissioners,
This letter is to request a public events permit. Covenant Hospice, along with Superior Residences and
Covenant Alzheimer’s Services will be hosting a 5k Run/Walk on November 15™ to raise money for their
Covenant Alzheimer’s Services affiliate. The event will be approximately 4 hours in duration. The event
will begin with Race Day Registration at 8:00am and will conclude after the awards ceremony around
12:00pm. The event coordinators are Elizabeth Rambow, Superior Residences (850) 499-3390, Rachel
Mayew (850) 428-0605 and Melita Brantley (850) 699-7777. The event will be run out of the back of the
parking lot of the Bluewater Fitness Center which is off of 4591 Highway 20, Niceville, Florida. All of the
money raised at this event will stay local to help area families in Okaloosa and Walton counties dealing
with Alzheimer’s and other forms of dementia.
Our route is very simple. [t will begin on the back driveway of Bluewater Bay Fitness Center Parking Lot
which is down from the corner of John Sims Parkway and White Point Road. The runners will go down
the bicycle lane of White Point Road and turn around and come back the same way.
We have received approval from Major Greg Gaddis at the Sheriff’'s Department and the posse is on
board to monitor and give support to our event. We have our liability insurance with $1,000,000.00
aggregate to cover any injuries or property damages.
Just a little bit of history about our organization. In 1996, Alzheimer's Support Group incorporated as a
nonprofit 501(c)(3) organization and changed its name to Alzheimer's Family Services, Inc., continuing to
provide valuable services to the community. To further strengthen its capabilities, in 2006 AFS became a
subsidiary of Covenant Hospice. In 2014, Alzheimer’s Family Services has changed their names to
Covenant Alzheimer’s Services. Covenant Alzheimer's Services, Inc. (CAS) provides support to families
coping with Alzheimer's disease. As caregivers and providers themselves, CAS understands how this
disease can affect a family's emotional, physical, and financial well-being. Through services, resources,
and education, AFS can help provide quality care for loved ones and respite for caregivers. As an affiliate
of Covenant Hospice, Inc., Covenant Alzheimer’s Services is able to provide enhanced community
education, support, and services for families dealing with Alzheimer's disease and other forms of
dementia.
Covenant Alzheimer’s Services, Inc. is a private, not-for-profit organization recognized under Internal
Revenue Service (IRS) Code 501(c)(3). AFS serves Escambia, Santa Rosa, Okaloosa, and Walton counties
in Northwest Florida and Escambia and Baldwin counties in Alabama. Alzheimer’s Family Services, Inc.
provides all of its services through private donations and grants. All funds donated are tax deductible
and remain in the local area. CFC #19552.

Services of Covent Alzheimer’s Services
® Project Lifesaver - Project Lifesaver provides a transmitting device worn by patients to quickly locate and safely return
wandering Alzheimer's patients to their homes. The program works in conjunction with local law enforcement, fire
departments, and search and rescue teams. The patient is fitted with a transmitter no bigger than a watch, which is
worn on the wrist or ankle. Should the patient wander off, the caregiver can simply call 911 to begin a search. The
program includes a monthly monitoring/maintenance fee, which can be assessed on a sliding scale for those on a fixed
budget.
® Education and Workshops - AFS recognizes the need to educate the community, caregivers, and health care
professionals about Alzheimer's disease. To increase awareness and knowledge, AFS hosts workshops and seminars,
fulfills requests for speaking engagements, participates in health fairs, television and radio shows and more.
® Support Groups - AFS offers monthly support groups in Escambia, Santa Rosa, Okaloosa, and Walton counties in
Florida, as well as Escambia and Baldwin counties in Alabama, for families affected by Alzheimer’s disease.
® Senior Fairs and Health Fairs - Awareness is a vital objective of AFS. To reach those who need its services, booths at
senior fairs and health fairs are staffed with knowledgeable volunteers, distributing valuable information to persons who
might otherwise not know where to seek help.
® Volunteer Services - Volunteers are the driving force behind all programs and projects, helping to determine the success
of Alzheimer’s Family Services. Nothing is accomplished through this organization without the assistance of volunteers.
The Alzheimer's Caregiver Assistance Reaching out and Educating (CARE) Volunteer program places trained
volunteers with community caregivers for additional support.
Newsletter - AFS stays in touch with families served through its informative newsletter, The Caring Times. This publication,
containing current research on the disease, caregiver tips, and important dates, has a circulation of more than 1,800
each month.
e Lending Library - AFS houses a lending library of books, periodicals, videos, and activities available to any person in
need of education and understanding.
e Counseling and Referral Service - A major function of AFS, calls and visits are received daily from persons seeking help
with placement, financial or legal assistance, medical information, and support. One-on-one and group counseling are
available upon request and at no charge.
e Loan and Supply Closet - AFS maintains an inventory of non-medical equipment (shower chairs, wheelchairs, walkers,
etc) for loan. Bed and chair pads, incontinent briefs, and dietary supplements may also be available.
The Caregiver's Handbook - AFS provides the Caregiver's Handbook, containing signs and symptoms of Alzheimer's
disease, caregiver advice, contact information and resources, at no charge to anyone who requests it.
e NMemory Screenings - Alzheimer's Family Services conducts Memory Screenings at various locations throughout its
service area. Although appointments are required, memory screenings are available at no charge.
