Motion to approve Service Agreement #21-24 Jamie Fields for an amount not to exceed $7,700.00, effective July 20, 2020 through June 30, 2021. Public Content Request approval of Service Agreement #21-24 between Jamie Fields and The School Board of Okaloosa County, FL, on behalf of Choctawhatchee HS, to provide Monthly Clinician Services. The Provider will be paid $400.00 per month for the months of July-October, and $700.00 per month for the months of November through June 2021 for a total of $7,200.00. Any additional choreography/routines/tryouts will be based on $25.00 per hour, for a total of $500.00, for a total contract amount not to exceed $7,700.00.
