Dear Ms. Cardreon,
This is the 5th year for the Crestview Pregnancy Center 5K (formerly known as the
Mosaic 5K). The race will occur on Saturday 13 April 2013 at 8:30am, starting and
ending at Old Spanish Trail Park. One hundred percent of the proceeds from the race will
benefit the Crestview Pregnancy Center.
The Crestview Pregnancy Center (CPC) is a not-for-profit organization which has been
assisting Crestview women in crisis pregnancies since 1999. The Center provides
services including health counseling, medical services, and The Sexual Integrity
Program.
The purpose of my letter is to request approval for the event from the Okaloosa County
Commissioner’s Office. You will find the necessary paperwork enclosed. As per the
permit request information sheet, I have confirmation from Mr. Greg Gaddis of the
Okaloosa Sheriff's Office and Mr. Shane Stewart’s confirmation of exemption for the
Express Assumption of Risk Class.
