Agenda Item Number: RG - 18
CITY OF FORT WALTON BEACH
MEMORANDUM
DATE: January 9, 2007
TO: Joyce Shanahan, City Manager
FROM: Mark Wiltsie, Director of Human Resources
SUBJECT: Renewal of Medical Insurance Program
Back2round:
As part of the City' s total benefit package, full-time employees can participate in the City' s
medical insurance program. Currently, the City contributes 75% of the premium while
employees contribute 25% through payroll deduction. In addition, retirees are eligible to
continue their medical insurance coverage by paying 100% of the premium. Employees who
terminate from City employment may elect to participate in the medical plan under the
provisions of the Consolidated Omnibus Budget Reconciliation Act ( COBRA). This federal law
provides for the continuation of coverage ( typically 18 months) and requires the former
employee to pay 100% of the premium.
In January 2001, as a result of a Request for Proposal ( RFP), the City entered into an agreement
with Blue Cross/Blue Shield ( BC/BS) of Florida to provide medical insurance coverage for our
employees, retirees and their families. Since that time the City has renewed its coverage with
BC/BS on an annual basis. The BC/BS insurance program currently provides for three ( 3) levels
of coverage and associated premiums of which employees, retirees and COBRA participants can
select either of the following:
1752 Base Plan
In February 2005, the City adopted this plan as the base plan for our employees and retirees
which also can be elected by COBRA participants. This is a BC/BS Blue Options plan and
provides for reasonable coverage levels and premiums consistent with the local market.
702 Buy Up Plan
This is BC/ BS' s Blue Choice plan and offers higher coverage levels and provides for a broader
network of physicians than the Blue Options plan. Employees who elect this plan are required to
pay the entire cost of the upgrade. In other words the City' s share of the premium remains the
same as the 1752 Base Plan. As mentioned above, retiree' s or COBRA participants are required
to pay 100% of the premium regardless of the which program they elect.
1352 Buy Down Plan
This Blue Options plan offers coverage levels which are somewhat less than the Base Plan and as
such, the premiums are relatively lower.

Discussion:
In anticipation of the upcoming renewal of the City' s medical insurance program effective
February 1, 2007, staff met with our Blue Cross / Blue Shield ( BC/BS) of Florida representative
Lisa Dixon in December 2006 to discuss our claims history and utilization of the plan. At that
point, BC/BS underwriters rated our programs as requiring a 11. 2% increase for renewal.
Following further discussions with Ms Dixon and a re- submittal to underwriting for
consideration, an 8% increase resulted. Although I believe this is a reasonable increase based on
the last 12 month utilization of the plans and the continuing escalating costs in the medical
industry, staff requested BC/BS underwriters to consider further reductions in the premium
renewal if changes in plan coverages were made.
The chart below identifies each ofthe three ( 3) plans, the changes in plan design for each and the
current and proposed premiums:

for the increases associated with the medical insurance renewal.

agreement with Blue Cross/Blue Shield of Florida as proposed in this staff report.
