BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: July 1, 2014 TO: Honorable Chairman and Members of the Board FROM: John Hofstad SUBJECT: 12th Avenue Sidewalk CEI Services Final Payment DEPARTMENT: Public Works BCC DISTRICT: Two STATEMENT OF ISSUE: On January 6, 2014 Okaloosa County entered into a contract with Genesis CEI Services, LLC to perform Construction Engineering & Inspection services for the 12th Avenue Sidewalk LAP project in Shalimar. The negotiated contract was a Not-To-Exceed amount of $17,010.26. The project was substantially complete as of April 24, 2014 and the Contractor is in the process of submitting documentation to close out the project. BACKGROUND AND ANALYSIS: The 12th Ave. Sidewalk addition project consisted of adding pedestrian sidewalk along the south side of 12th Avenue from State Road 85 (Eglin Parkway) to 12th Street in Shalimar, FL. The project was funded via LAP Agreement from the Florida Department of Transportation. There was no funding match required by Okaloosa County with this Agreement. Genesis CEI Services, LLC has requested final payment be made for services performed.
