BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: January 3, 2017 TO: Honorable Chairman and Members of the Board FROM: Tracy Stage SUBJECT: Change Order No. 1 for Airport Guide Signage DEPARTMENT: Airport BCC DISTRICT: 2 STATEMENT OF ISSUE: The Airports Department requests the Board of County Commissioners approve Change Order No. 1 with Gulf Coast Traffic Engineers, Inc. to extend the construction time by 184 days on Contract No. C16-2450-AP and also to add $119,000.00 to the construction contract in order to complete the addition of a new guide sign on Highway 85 just north of the Destin - Fort Walton Beach Airport. BACKGROUND: The construction contract for the Addition of Guide Signage was issued on September 19, 2016 and the Notice-to-Proceed was issued on September 21, 2016. The project consists of all labor, materials, machinery, tools, equipment and other means of construction necessary and incidental to the completion of the work including fabrication and installation of sign overlays on 21 directional signs in Okaloosa County. The project is funded by Florida Department of Transportation (FDOT) and Local Match (County). See Funding breakdown below. FDOT $75,000.00 Local Match $75,000.00 Total Funding $150,000.00 Additional supporting documentation attached to this Agenda Request:  Change Order No. 1 proposes to construct a new cantilever guide sign on the north side of westbound Highway 85 just north of the Destin-Fort Walton Beach Airport to provide clear guidance to travelers for the turn-off to Shalimar via Hwy 85 and Crestview via Hwy 123. Fabrication of the metal sign structure is estimated to take 18 weeks and the sign installation is estimated to take approximately three weeks. The design drawings are attached as Attachment A. This change order will add 184 calendar days to the construction contract for fabrication and installation of the new sign structure giving the project a new substantial completion date of June 20, 2017 and a new contract expiration date of July 10, 2017. Change Order No. 1 will also add $119,000.00 to the construction contract for this work giving a new contract price of $137,915.00. This change order has been approved by Airports Director, Tracy Stage, Project Engineer, John Collins, and Gulf Coast Traffic Engineer’s President, Blair Crooke. It has also been approved by FDOT District 3 Program Manager, Philip Deal.  The County internal coordination by the Purchasing Department and Legal Department.   A location depiction aerial that shows an example of what the guide sign will look like as well as the sign details once it is complete. This aerial will be shown as a PowerPoint Slide during the agenda presentation.  The original contract date is written incorrectly on the Change Order and is notated with a strike-through and the initials of Wendy Cain, the individual that made the correction.
