BOARD OF COUNTY COMMISSIONERS AGENDA REQUEST DATE: April 5, 2016 TO: Honorable Chairman and Members of the Board FROM: Elliot Kampert SUBJECT: Approval of Contract with Logisticare, LLC DEPARTMENT: Growth Management BCC DISTRICT: All STATEMENT OF ISSUE: In order to provide non-emergency medical transportation for trips originating in Okaloosa County, it is necessary for the County to enter into an agreement with the state's contracted non-emergency medical transportation coordinator, Logisticare Solutions, LLC. BACKGROUND: Prior to the passage of the Affordable Care Act, the state Commission for Transportation Disadvantaged contracted directly with communities to provide non-emergency medical transportation using Medicaid funds. However, subsequent to passage of the Act, the state entered into a contact with Logisticare Solutions, LLC (Logisticare), to coordinate the provision of such transportation. Accordingly, Logisticare entered into a contract with the County's previous public transportation provider. On October 1, 2015 Okaloosa County's public transportation services, including non-emergency medical transportation, transitioned to a new provider, Maruti Fleet and Management, LLC. At that time, the existing contract between the County's prior transportation provider and Logisticare became void, and so to ensure the continued provision of non-emergency medical transportation service, the County entered into an emergency contract with Logisticare. The attached contract will supersede the emergency contract under which services have been provided. OPTIONS: The Board may approve or reject the attached contract. RECOMMENDATIONS: Staff recommends the following: 1. That the Board of County Commissioners approve and authorize the Chairman to sign the attached agreement with Logisticare Solutions, LLC., including all forms and attachments, and 2. That the Board of County Commissioners authorize the Chairman to initial the revisions set forth within the Software User Agreement, and sign the Software User Agreement; and 3. That a copy of the County's current W-9 form be attached both to the original agreement to be retained by the County and the original agreement to be sent to Logisticare; 4. That a completed Electronic Funds Transfer Authorization Agreement including all requisite information and voided check be attached both to the original agreement to be retained by the County and the original agreement to be sent to Logisticare; and 5. That, as public transportation vehicles change, Maruti Fleet and Management. LLC., be authorized to update and furnish to Logisticare the revised vehicle inventory without necessitating amendment to this agreement by the Board of County Commissioners provided that copies of the updated inventory are provided to the Okaloosa County Purchasing Department as well as the Contracts and Grants office of the Clerk of the Circuit Court. RECOMMENDED BY: APPROVED BY: John Hofstad, County Administrator
