STATEMENT OF ISSUE: Request approval of sole source for Georgia State Association
Executives (GSAE) for Peter Mayer task order 79.
BACKGROUND: The Task Order Approval Form for Peter Mayer task order 79 in the amount
of $4,050 for digital banners on the GSAE website was approved by the Purchasing Department
on September 18, 2013, but a sole source form was not prepared with the task order.
The task order was executed prior to the current Purchasing Manager, so Board approval is
requested for the sole source form. Payment cannot be processed without a sole source form.
OPTIONS: Approve or Disapprove
RECOMMENDATION: Approve sole source for Georgia State Association Executives
(GSAE) for Peter Mayer task order 79 and authorize the Purchasing Manager to sign the form.
