RESOLUTION 2008-47 A RESOLUTION OF THE CITY OF FORT WALTON BEACH, FLORIDA, AMENDING SECTION 2 OF RESOLUTION 2008-06 WHICH ADOPTED POLICIES FOR THE COLLECTION AND USE OF SOCIAL. SECURITY . NUMBERS; AND SETTING AN EFFECTIVE DATE.' \' BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF FORT WALTON BEACH, FLORIDA AS FOLLOWS: 1. On January 22, 2008, the City Council adopted Resolution 2008- 06 to satisfy the requirements of section 119. 07, Florida Statutes, regarding the collection of social security numbers. Through implementation of Resolution 2008- 06, the City Police Department has found that at the time of collection of the social security number, not all individuals would or were available and capable to sign the use disclosure. Section 119.07( 5)( a)( 3), Florida Statutes requires the City to provide a copy of a disclosure of authorized use at the time of collection but does not require that the individual sign it. Therefore, the City Council hereby amends Section 2, of Resolution 2008- 06 as follows: J the time that a social security number is requested from an indi~ Jidual, a disclosure of the purposes for collection, as stated in this resolution, shall be signed by the indi~.' idual. When a City department collects a social security number from an individual, the City department shall provide to the individual a copy of a disclosure stating the authorized purposes for collection. 2.
