C-12-1960 DEPARTMENT: Public Works BCC DISTRICT: 1,3 STATEMENT OF ISSUE: Public Works provides a list annually to the Board for consideration and approval. Task order 3 for contract C12-1960 was presented in February of 2015 and subsequently approved. This is the final pay request for task order 3 for contract C12-1960. BACKGROUND: APS has been contracted by Okaloosa County to provide pavement preservation alternatives since 2012 and have exhausted all available extensions. The BCC on October 6, 2015 approved the amendment for a change in cost to the original contract whereas two line items were not increased, Mobilization and MOT. OPTIONS: Approve or Disapprove RECOMMENDATIONS: Approve final payment on contract C12-1960-PW in the amount of $46,607.76 RECOMMENDED BY: APPROVED BY: John Hofstad, County Administrator
