changes include the addition of a third plan, as well as, the expansion of all plans to four tiers (single, employee + child(ren), employee +
spouse and family) for the 2019 plan year.
The Group Flexible Benefits (FSA) Plan and Healthcare Reimbursement Arrangement (HRA) Agreement administered by Lockard &
Williams Insurance Services, P.A. are also presented for approval of a one year, annually renewable extension of the original
agreements (attached for reference). Execution of the attached letters will extend the agreements for the 2019 plan year per Section 2
TERM of the original agreements.
The annual employee maximum contribution limit of the FSA plan has been increased from $2,600 to $2,650 in accordance with the
most recently enacted changes to the Internal Revenue Code (IRC) Section 125(i). Recall that, during the 2018 renewal cycle, the

The School Health Services Plan is a two-year document designed to facilitate the establishment of local strategies to implement
activities mandated by law to meet the health of Florida's student and improve their chances for success in school. Although Department
of Health in Okaloosa County takes the lead on preparing this document, the law requires that this plan be collaboratively developed and
approved by the county health department (CHD), the local school district (LEA), contracted health provider (PSA) and the local school
health advisory committee (SHAC) and to include a process for data collection that can be evaluated. The entities involved have met
and have been a part of the development of this document.
For additional information, please contact Teri Schroeder, 833- 3108.
