PAVEMENT REHABILITATION OF MARTIN LUTHER KING JR. BOULEVARD
AND GREEN ACRES ROAD
INTRODUCTION
This scope describes the Design, Geotechnical, Surveying and Environmental services HDR will
perform related to pavement rehabilitation on Martin Luther King Jr. Blvd. (MLK), and Green
Acres Road in Fort Walton Beach, FL. The project limits are MLK from Butler Drive to Green
Acres Road, and Green Acres Road from MLK to Lewis Turner Boulevard, a distance of 3.1
miles. An exception exists for the curve at MLK and Freedom Way from approximately 400’
south to 950’ north of Freedom Way, which will be reconstructed by others. A southbound left
turn lane is proposed on MLK at Florence Avenue.
The pavement surface of MLK and Green Acres Road is failing throughout the project limits,
and initial investigation suggests varying types and severity of failure. In this regard, the project
will be phased as follows:
e Phase I consists of geotechnical analysis of the roadway, pavement design, cost
estimating, and recommendations for corrective measures.
e Phase II includes the development of design plans to implement recommended corrective
measures and detail the left turn lane at Florence Avenue.
Assumptions:
1. Phase I analysis will evaluate corrective measures such as milling and resurfacing, and
full depth reclamation. No design will be performed in Phase I other than pavement
design.
2 Phase II will involve the preparation of design plans to include plan view only. No
easements or right-of-way acquisition is required for the project. Correction of roadside
slopes and other roadside safety issues are not included.
Okaloosa County will supply AADT counts for pavement design.
No wetland impacts are anticipated.
5. This project is assumed to qualify for an ERP exemption under 62-346.051(4)(c) F.A.C.
Services required to provide improvements beyond the qualification for this exemption
are not included in this Scope of Services.
This project is assumed to comply with the Okaloosa County LDC without ponds.
100-Scale Plan Sheets will be prepared for the full length of the project, and no profiles.
A FDOT connection permit may be required for this project at Beal Parkway.
No special driveway profile designs are required for this project. Driveways will be
handled using FDOT standards and/or typical sections to the extent impacted.
10. The following standards and specifications shall be used on the project:
a. Florida Department of Transportation Design Standards, 2013
b. Florida Department of Transportation Standard Specifications for Road and
Bridge Construction, 2013
c. Florida Greenbook, 2011
11. Construction observation is not included in this scope and fee.

TASK 2 —- GEOTECHNICAL INVESTIGATION (PHASE I)
Subtask 2-1 Conduct Ground Penetrating Radar (GPR) Survey
Objective:

Environmental and Geotechnical Specialist’s Inc. (EGS) high speed profiling unit to
identify areas of subgrade/pavement deficiencies.
Activities:
=" EGS will conduct a GPR and Geotechnical Survey of the existing roadway in the
following manner:
a. Runa GPR Transect line in each shoulder and each travel lane (the EGS GPR
unit is vehicle mounted such that data can be gathered near existing roadway
speeds to reduce disruption to traffic).
b. Install 2 pavement cores and a 10 feet deep auger boring for each mile of
roadway (total of 6 cores).
c. At each core install 1-10 feet deep auger boring to evaluate subgrade,
groundwater, and general subsurface conditions (6 borings x 10 feet/boring =
60 feet).
d. Grout soil borings and patch roadway surface.
= EGS will conduct the following Laboratory testing:
a. Perform 1 grain-size classification per 10 feet of boring (6 tests).
b. Perform 1 plasticity test per 20 feet of soil boring (3 tests).

TASK 5 —- SURVEY (PHASE IT)
Topographical survey will be conducted by Nobles Consulting Group, Inc. (NCG).
Subtask 5-1 Alignment
NCG will determine an alignment along 3.1 miles of MLK Drive and Green Acres
Road from a legal description on file with the Engineering Department of Okaloosa
County and a AutoCadd dwg in NCG’s files.
NCG will utilize existing found control in the existing database as previously
provided from Okaloosa County Engineering Dept.
NCG will re-check and re-tie secondary control points as set on prior project of MLK.
NCG will extend the alignment along MLK to the South for 2800 linear feet, to south
of the transition point at NW Butler Drive. NCG will back station the existing
alignment, so it matches with existing data on MLK.
NCG will create a new alignment from the North end of MLK Drive, East along
Green Acres Road to the centerline of Lewis Turner Blvd, consisting of approx. 4100
linear feet. NCG will create a station equation at the intersection of MLK Drive and
Green Acres Rd.
NCG will not have the Right of Way determined with this alignment. It will be an
approximation or referenced in by Okaloosa County GIS shape files.
NCG will not stake alignment or place reference points in the field.
Subtask 5-2 Horizontal Location Survey (2.8 Miles)
NCG will
