STATEMENT OF ISSUE: [PUR] Seeking approval to extend contract with Jimmy Henderson
III Construction (C14-2171-HD) retroactively from July 21, 2014 and execute Change Order No.
6.
BACKGROUND & ANALYSIS: On April 04, 2014, the County entered into a contract with
Jimmy Henderson III Construction to repair and renovate a modular building at the Okaloosa
County Health Department. Change Order No. 4 was approved on July 1, 2014 to extend the
contract from the original completion date of May 16, 2014 to July 21, 2014. (Change Order #5
added costs to the project with a resulting total contract amount of $109,986. No time was
added.) Due to extenuating circumstances the project was not completed by the expiration date
of July 21, 2014. Staff is requesting the Board approve extending the contract from July 21,
2014 to the new anticipated date of completion of August 11, 2014 and authorize the Chairman
to sign Change Order No. 6.
OPTIONS: Approve/Deny request to extend the Contract (C14-2171-HD) with Jimmy
Henderson III Construction from July 21, 2014 through August 11, 2014 and authorize the
Chairman to sign Change Order No. 6.
RECOMMENDATION: Staff recommends approval to extend the Contract (C14-2171-HD)
with Jimmy Henderson III Construction from July 21, 2014 through August 11, 2014 and
authorize the Chairman to sign Change Order No. 6.
