PURCHASING DEPARTMENT WILLIAM J. WORLDS PURCHASING AGENT Email. wworlds@fwb.org November 5, 2009 RE: Minutes From Evaluation Committee Meeting For iTB 10 -001, Front Loader Refuse Truck ATTENDEES: William Worlds, Purchasing Agent, City of FWB Allen Lassiter, Operations Manager, City of FWB Henry Hill, Fleet Superintendent, City of FWB Jerry Gunn, Solid Waste Supervisor, City of FWB The meeting was called to order at 10:00 A.M. by William Worlds All Committee members had previously reviewed all bids submitted prior to the meeting. Three of the seven bids received were deemed "Non- Responsive" because the bidders did not complete and return the required Rust-Proofing Statement Form. This form required all bidders to indicate the specific rust-proofing materials that will be used, required the bidder to indicate the name and address of the company that will be performing the rust-proofing, warranty period for the rust-proofing, and whether the bidder will be able to deliver a rust -proofing warranty certificate upon delivery of the vehicle. The three bidders that did not complete and include the Rust -Proofing Statement Form and whose bids were rejected are: RDK Truck Sales Action Truck Center Truck Equipment Sales None of the bidders included the Statement of Experience and Qualifications for application of the Rust-proofing as requested on the Rust-proofing Statement Form. The Committee recommended that this requirement be waived for all bidders as a technicality as provided in the bid document (Notice to Bidders, page 33). After a discussion and review of the four bids deemed " Responsive" to the bid specifications, and evaluation criteria, Ingram Equipment Sales was identified as the bidder with the highest overall score. The Committee unanimously agreed that the recommendation of award to City Council be made for Ingram Equipment Sales. The summary of Scores for each Responsive bidder is included on page two of these minutes. 12- CityAmerica`s of Fort Walton Beach Most Beautiful Beaches 143 Hollywood Blvd. NW • P. O.
