This is our second event with plans for continuing every spring. The purpose of this letter is to solicit your support through the sponsorship of: one or more golf holes at our tournament; sponsorship of team; a combination of both team and hole sign; or by donation of merchandise or services. This tournament will provide funds to supplement the band program’s school board funding providing opportunities for students to fully develop their musical talents and participate in events and competitions that the funded budget doesn’t include. Additionally, these funds provide for repairs to equipment, transportation expenses to events/contests, uniform replacement and repair. The amount of additional funds necessary to run a band program yearly is more than $50,000 above official funding. Simple hole sponsorship only costs a meager 5100 for a single hole. Each hole may have multiple sponsors but, we are limiting sponsorship to a maximum of 6 sponsor signs per tee box. If you would rather, you can sponsor a full team’s entry for $100, or sponsor a team and a hole w/sign for $200, A sign with your logo and/or appropriate wording would grace the tee box for each hole you choose to sponsor, If you feel that you are unable to be a sponsor but are wanting to help, we are in need of donated items and/or services for auction, prizes for closest to the pin and other various golf skills’ challenges, or a simple monetary donation to go to the Band Program. Please indicate your preference on the enclosed Sponsor Form. All donations will be prominently recognized both before and during the event. We also invite you to participate with us, not only in a fun round of golf, but in the fellowship and fun that is provided. If you desire to sponsor a hole or a team, please remit your sponsorship form and payment to the above address payable to: Baker Band Boosters. If you prefer you may contact me or the Tournament Chairman to arrange for us to come and pick up your donations. Oafehy. Setond Chanter. Dear Okaioosa County Sheriff's Office, Thanks to the generous donation from your organization, hundreds of animals in need were provided for at Alaqua Animal Refuge in 2012. Your commitment to helping abused and neglected animals receive Shelter, Safety, and Second Chances is appreciated by all of us who care for them. Thanks to donors like you, Alaqua Animal Refuge has provided a safe haven for over 8,000 animals since its inception just five short years ago. This year’s accomplishments also include representing the state of Florida on Capital Hill to lobby for more humane animal welfare laws, partnering with the Florida Sheriff's Association and the State Attorney’s offices to bring our animal cruelty investigation and prosecution seminars state wide, and developing a program to utilize prison labor to train rescued canines as therapy dogs for families who can not afford them. We are a small organization without the resources available to large, national humane groups. Yet, we have grown to be a nationally recognized rescue, and we are called upon daily to locally provide support in the Southeast United States where the national organizations don’t reach. Your continued support of Alaqua is vital to the health and wellbeing of our entire organization. i'm asking for your continued support that gives so many animals, and humans alike, a second chance. We invite you to take a tour of our campus and meet some of the 250 animais who are currently in residence. We are always proud to show off our facility and have our supporters meet the animals we care for with the help of your generosity. | am always available to answer any questions you may have. Located on 10 acres in Freeport, Florida, the refuge was formed by Florida resident Laurie Hood in 2007 when she discovered that countless animals were being euthanized due to the flack of a no-kill shelter/adoption organization in the area. Since that time, Alaqua Animal Refuge has helped find homes for more than 8,000 animals, serves as a true refuge for more than 250 animals at any given time, and has become a recognized national leader in animal welfare issues. From crisis intervention training to Baker Act and beyond, your office has worked to keep our community safe. As you may be aware, State funding reductions have closed Okaloosa mental health group homes while uninsured adults in need of psychiatric appointments are being turned away due to the same funding decisions. Decreases in mental health treatment and shelters frequently leads to an increase of law enforcement involvement with mental health crises. The Mental Health Association works to assist our community residents by purchasing emergency psychiatric medication for persons in need. Our program helps individuals remain psychiatrically stable, lowers jail and hospital recidivism rates, while reducing law enforcement crises responses. The Mental Health Association is requesting five thousand dollars from the Okaloosa County State Forfeiture Program to keep our community a safe place for all. Our volunteers see firsthand many acts of true compassion by these heroic professionals. The volunteers themselves have selected the most outstanding of those nominated throughout the year to receive these awards. On Saturday, January 19, 2013, the TIP Program will host this gala event at the Hurlburt Soundside Club at Hurlburt AFB-in Mary Esther. The event, including dinner and awards, will begin at 6:00pm with bidding on silent auction items, the proceeds of which will be used as a means for raising funds for our local tri-county TIP Chapter. We are asking local businesses, individuals, and agencies for support by sponsoring one or more tables (for 10), for $500.00 each. Table donors will be recognized on a list of sponsors displayed at the gala, by announcement during the evening, and will also be listed on our sponsor recognition page online. Donations may be tax deductible, as TIP is a 501(C)(3) organization. Individual tickets can also be purchased for $50.00 each. We would love to have you in attendance to show community support for these heroic men and women. This is truly an exciting event, and we look forward to a night of fun, fellowship, and most importantly, recognition of the exceptional work done by our emergency responder personnel. We expect considerable media coverage, and representatives from TIP chapters throughout the USA will also be in attendance. The purpose of this letter is two-fold. The first purpose is to thank you for the years of support in the community and your assistance in providing social care for our community. Sincere thanks for considering it a great idea to reinvest seized money and property from criminal activities, back into the same neighborhoods and children that these activities have had its greatest impact. 2. The second purpose is to request your support again for the upcoming school year. Research overwhelmingly demonstrates that parent involvement in children's learning is positively related to achievement. Furthermore, research shows that the more intensively parents are involved in their children’s learning; the more beneficial are the achievement effects. This holds true for all types of parent involvement in children's learning and for all types and ages of students. In light of this knowledge, we are again asking for your support of the Parental Involvement Program at Choctawhatchee High School. All of the teachers who participate in this program are volunteers, therefore we attempt to provide a small incentive reward (paper, dry erase markers, etc) each month to each teacher as well as a “Kick-off dinner and Graduation dinner for the teachers, students, and the students’ family. Sheriff Ashley, here are results of the “My Team” program that you supported in the 2011-2012 school year. e 97% of students either stayed on the same level or increased at least one level on the Reading FCAT!!! In fact, only one student actually decreased a level on the Reading FCAT. « 45% of students improved attendance from the previous year. One student reduced. absences from 22 to 8. ¢ 21% improved discipline. One student reduced discipline from 9 referrals in 8" grade to only 2 in 9" grade. 3, The purpose of this program is to: a. Improve FCAT reading scores for all participating students b. Build a relationship between the student, the parent and the school c. Improve parental attitude and rapport with the school Okaloosa & Walton And the King will answer and say to them, ‘Assuredly, | say fo you, inasmuch as you did it to one of the least of these My brethren, you did it to Me.’ Matthew 28:40 (NJKV) d. Improve parents concept of themselves and the vital role that they play in the lives of their children 1) 30 teachers x 9 months x $7.00 = $1,890 (This equates to $63 per teacher each school year) 2) Kick-off Dinner and Graduation Dinner = 150 (teachers, students, and the student’s family) x $6.00 per meal x 2 dinners ($900) = $1,800 3) Total Sponsorship: $3,680.00 . 4, Again, to let the community know your support, we encourage you to consider our proposal in taking the first steps on the long road toward helping our students achieve academic excellence, Sincerely, CK > Lin Mack Executive Director S4P Synergy Inc Okaloosa County Sheriff's Office Application for Florida Contraband Forfeiture Monies Saction 932.7053(5ic}, Florida Statutes i Agency or Organization requesting funds: Narne: S4P Synergy Inc Address: 24 Bass Ave SW City: Fort Walton Beach State: FL Zip Code: 32548 Contact Person: _Lin Mack Telephone: (850) 362-6977 Fax: _(850) 833-3005 Email Address: s4psynergy@striving.gccoxmail.com 2.
