Recommended Action : Motion to approve Addition of the
Level2 Type 2 Diabetes Program
offered by United Healthcare
Public Content
Level2 is an evidenced-based, specialty care program designed to address the unique challenges faced by employees living with Type 2
diabetes. Eligible employees will receive a continuous glucose monitor (CGM) to enable them to see how glucose is impacted by the
choices they make. A clinical care team will monitor CGM-provided data and apply a step-by-step method to build new habits and
enable better outcomes.
United Healthcare has identified 371 members that are living with type 2 diabetes – an increase of 6% over the previous year. Claims
costs for these individuals averaged $1,120 per member per month (PMPM) in 2022 and are projected to increase to $1,570 without
intervention.
The cost of the program is $225 PMPM (participants only) and includes performance guarantees. OCSD will be reimbursed for shortfall if
claims are not reduced by at least the cost of program. If a claims reduction greater than $15 over expected is achieved, OCSD and
UHC will share the savings over that threshold.

Elementary School and Baker School Additions.
Scope of Work:
New K-8 School - Construction of a new 1,100 student K-8 School in Crestview on 80 acres of District-owned
land will be completed under this task order. This school will contain all required components for successful
operation such as a Gymnasium with PE/Athletic Locker Rooms, Band /Chorus Rooms, Administration,
Media/Innovation Center, CTE/Science/Art Wing, and Cafeteria/Kitchen in addition to the two-story classroom
wings that will house the K-8 students. The school site will provide the requisite parking, traffic drop-off/pickup
queuing for parents and buses, and athletic/recess fields for staff and student use. In addition to traditional
classrooms, an appropriate quantity of ESE Classrooms are also designed into the school plan. The Cafeteria is
designed as an Enhanced Hurricane Protection Area (EHPA) Emergency Shelter as required by code.
Destin Elementary School 3/4/5 Center Project - This project consists of a two-story thirty (30) classroom
addition at Destin Elementary on the grounds of the current school. This addition will house 3rd, 4th, and 5th
grade students and also includes a Multi-purpose Room and Administrative Suite that will serve the entire
campus. A stand-alone Restroom/Teacher’s Lounge/Storage building will also be provided that is easily
accessible from the main campus and specifically is designed to serve the existing Cafetorium during public
events and student use during regular school hours. A new drop-off/pickup roadway will be provided on land
recently purchased by the District that is directly adjacent and South of the addition. This will relieve the existing
traffic issues associated with the current drop-off road configuration. Additional parking will also be provided for
staff and visitors to the school. Sitework includes construction of all required walkways with requisite canopies,
a new PE pavilion and relocated/new playground that will be established once the project is complete and
portables are removed.

the North end of the Baker campus adjacent to the parent drop-off/pickup ramp. The building is designed for
secondary education and contains gang restrooms that can serve other areas of the campus. The building will
connect to the main campus via new sidewalks with covered canopies.
